A Case of Iatrogenic Penumothorax Developed after Total Laryngectomy & Bilateral Radical Neck Dissection and Its anesthetic Management




Abstract
We experienced one case of iatrogenic pneumothorax developed after total laryngectomy and bilateral radical neck dissection in 70-year-old male patient with laryngeal, hypopharynx cancer.
The operation was performed succesfully for about 9 hours with intensive monitoring include EKG, puls oxymetry, capnograph, arterial cannulation and NIBP. Patient was tansfered to ICU after operation without extubation. The 2 hours later,
clinical
findings and postoperative chest x-ray show the findings of pneumothorax definitely. Before the cardiopulmonary drangement was occured, proper management was applied to patients on the time of beginning of re-operation.
With the help of rapid and prompt decision and proper management to pneumothorax, patients recovered uneventfully without severe complication, and he was discharged after healing the tracheo-cutaneous fistula developed 10th days postoperatively.
In this case, we suggests that in the case of radical neck surgery, we should consider the chance of occurring pneumothorax, and should prepare anesthesia with aggressive & intensive monitoring and vigillance.
